You see the word in the target language. You attempt a translation from memory. The first encounter always counts as day 0.
If you recalled correctly, the next review is scheduled further in the future. If you forgot, it resets to tomorrow. The SM-2 algorithm determines the exact gap based on your response quality.
Each day, only the cards that are due appear in your review queue - no more, no less. You are never shown a word you just learned, and never asked to review a word you clearly know.
Learned Today → Review Tomorrow → Review in a Week → Review in Two Weeks → Review in a Month → Mastered. Mastered words exit the queue. Forgotten words reset.
